We are looking to recruit a Transitions Coordinator for Sense College Loughborough.  Sense College Loughborough is a specialist Further Education (FE) college, for young people with a wide range of disabilities. Our key aim is to prepare students to be as independent as possible to achieve their long term goals. 

The successful applicant will be responsible for ensuring students have an effective transition out of college.  This will involve chairing annual reviews, whilst providing support to students, parents/carers and colleagues so that all students are fully prepared for their best next steps. 

Key areas of activity include 

  • Preparing for and facilitating effective review meetings and ensuring completion of post review actions. 
  • Working with students to fully prepare them for their best next steps
  • Working with colleagues to support them to prepare students for their best next steps 
  • Working with partners and projects so students can access external transitions support
  • Ensuring all activity is monitored, recorded and the impact of enhanced transitions activity is evidenced. 

By coordinating outstanding transition planning you will play a critical role in ensuring we increase the number of students who achieve their aspirational goals.
